This means you can build your character from scratch, just like the quest Akara gives you for clearing the den of evil. This is a consumable item which you can craft using various essences which drop from Andarial in act 1, Duriel in act 2, Mephisto in act 3, Diablo in act 4 and Baal in act 5.
